Please fill out the following Registration Form to join our Aquatic Program. You will receive email confirmation of your acceptance into the program. Each applicant, or their legal guardian, must also provide a completed FMF Waiver Form. Please fill out the virtual FMF WAIVER FORM by clicking on the link below.
